Create a form from a document

Already have a questionnaire, application form, or intake form in a document? You do not need to recreate every question by hand.

Updated

With OG Forms, you can upload a PDF, Word document, or photo of a form. AI reads the content and turns it into an editable form draft.

This is useful when you want to move a paper form online, reuse an existing questionnaire, or turn a document into a form without starting from scratch.

What files can you upload?

OG Forms supports these file types:

File typeSupported formats
PDF.pdf
Word document.docx
Text files.txt, .md
Data files.csv, .tsv
Web documents.html
Rich text.rtf
ImagesPhotos and screenshots of forms

You can upload one file at a time. Each file must be no larger than 25 MB.

For scanned PDFs, OG Forms can read up to 8 pages. For text documents, the limit is about 200,000 characters, or roughly 50 pages.

Older Word .doc files and HEIC photos are not supported.

Creating a form from a document

You can create a form from a document in two places: the form creation screen or the builder chat.

Option 1: From the creation screen

Start from the create screen and choose the option to create a form from a document.

Upload your PDF, Word document, or photo of the form. OG Forms reads the file and uses AI to turn its content into a form.

Option 2: From the builder chat

You can also use an existing form builder chat to create a form from a document.

Open the builder chat and attach your document or photo. AI reads the content and generates a form draft based on it.

How OG Forms reads your document

OG Forms handles text documents and scanned documents differently.

Text based documents

For PDFs with selectable text and supported text formats, OG Forms reads the text from the file. It then processes the content before sending the relevant information to the AI model.

Scanned PDFs and photos

Scanned documents and photos do not always contain selectable text. In these cases, OG Forms reads the document pages as images using a vision model.

For scanned PDFs, up to 8 pages can be processed.

Review and edit your form

After processing the document, OG Forms creates an editable form draft.

The generated form is not published automatically. Review the questions and make any changes you need before sharing it with respondents.

Check that:

  • Every question from your document is included.
  • Question wording is correct.
  • Answer options are complete.
  • Required questions are marked correctly.
  • The form follows the order you want.

You can continue editing the draft in the builder. You can also use the AI form builder to create and edit forms through conversation.

What if your document is handwritten?

OG Forms can read text from photos and scanned documents using a vision model. However, handwriting recognition is not guaranteed.

Review handwritten questions carefully after the form is generated. You may need to correct words or enter unclear questions yourself.

Can I upload multiple documents at once?

No. Multi-file uploads are not supported yet. Upload one document at a time.
