Welcome to OG Forms
OG Forms helps you create forms, surveys and quizzes, collect responses, and manage everything in one place.
Updated
You can build a form from scratch, describe what you need and let AI create it, or import an existing Google Form. Once your form is ready, you can share it with others and manage responses from your dashboard.
Here is how to get started.
1. Create forms with AI or start from scratch
You do not need to add every question yourself. Just describe the form you want, and OG Forms can build it for you.
For example, you can ask OG Forms to:
- Create a client intake form for your business.
- Build a job application form for a new role.
- Create a customer feedback survey.
- Turn a document into a form.
You can also upload a PDF, Word document or image and use its contents to create a form.
If you prefer to build everything yourself, choose a blank form and add questions using the form builder.

2. Build forms, surveys and quizzes
OG Forms supports three types of forms. Each one is designed for a different purpose.
Forms
Use forms to collect information such as contact details, applications, registrations and customer enquiries.
Surveys
Use surveys to collect feedback and understand what people think. You can review NPS scores, rating averages and response themes.
Explore the AI survey generator to learn more.
Quizzes
Use quizzes to test knowledge and measure performance. You can set correct answers, assign points, add a passing score and show results to respondents.

3. Customize the look of your forms
You can change the design of your forms to match your brand or the purpose of the form.
OG Forms lets you customize:
- Colours: Change the background, text, buttons and borders.
- Fonts: Choose from the available font library.
- Layout: Adjust corners, alignment and column width.
- Images: Add a logo, cover image or background image.
Already have a website? You can enter its URL and let OG Forms find your brand colours, font and logo. You can then use the suggested design as a starting point.
4. Add rules to make forms smarter
Not every respondent needs to see the same questions. Conditional logic lets you show or hide questions based on someone's answers.
For example, imagine a customer feedback form that asks:
- How would you rate our service?
- If the respondent selects Poor, show a follow-up question asking what went wrong.
- If the respondent selects Good or Excellent, show a different follow-up question.
You can also use other features to make your forms more flexible:
- Answer piping: Use an answer from an earlier question in another part of your form.
- Form variables: Store and reuse values across your form.
- Calculated fields: Calculate values using respondents' answers.
- Hidden fields: Include information in a form without showing the field to respondents.
6. Connect OG Forms with your other tools
You can connect OG Forms to other services to make it easier to manage new submissions.
Available integrations include:
- Google Sheets: Send form responses to a spreadsheet.
- Google Drive: Connect your Google Drive account.
- Slack: Receive notifications about new submissions.
- Webhooks: Send form data to other services that support webhooks.
For example, you can send new responses to Google Sheets or receive a Slack notification when someone submits a form.
7. Choose a plan
OG Forms has two plans: Free and Pro.
Both plans include:
- Unlimited forms.
- Unlimited responses.
- Unlimited teammates.
- All question types.
- Conditional logic and calculated fields.
- Quizzes and scoring.
The main difference is the number of AI credits and available file storage.
| Feature | Free | Pro |
|---|---|---|
| Monthly price | $0 | $9 |
| AI credits per month | 25 | 250 |
| File storage | 100 MB | 1 GB |
You can build and edit forms manually even when you have used all your AI credits. Your existing forms and responses remain available.
