Connect Slack
You can connect Slack through the Integrations panel for your form.
Follow the connection flow to authorize OG Forms to send messages to your Slack workspace.
Choose where notifications go
After connecting Slack, choose the channel where you want to receive form submission notifications.
For example, you could send new lead notifications to your sales channel or client intake notifications to your operations channel.
Receive notifications when someone submits your form
Once the integration is set up, OG Forms sends a Slack notification when a new response is submitted.
Your team can see that a new submission has arrived and follow up without needing to keep the form dashboard open.
Track failed deliveries
OG Forms includes a delivery log for Slack notifications. If a delivery fails, the system can retry it.
You can also replay a failed delivery from the log. OG Forms sends an alert to the owner when an integration starts failing.
This helps your team spot connection problems instead of silently missing notifications.
